Unhappy about a public service at the Stratford Agriplex? Here's how to let us know

We want to know if you’re unhappy or have a complaint about a public service provided at the Stratford Agriplex. Your effort will give us the chance to address your issue. Here’s how to lodge a complaint.

Stage 1: Make your complaint directly to the front desk

You can do this in personor by phone.

  1. Immediate Action Required: If your complaint requires immediate attention, we will attempt to resolve it on the spot or as soon as possible.
  1. Action Can Wait: If our frontline staff determine that an action to the complaint can wait, we will ask you to complete a ‘Letter of Complaint’ (see below). Upon receipt of your letter, we will try to resolve your issue within 5 working days. We may respond to you by phone or in person to try and resolve the complaint. If you are unhappy with our response, you can ask us to consider further investigation to your complaint by activating Stage 2.

Stage 2: Complaint Investigation Request

At this stage, you will be required to tell us why you are requesting a further investigation of our response to your initial complaint. We will ask you to complete a ‘Complaint Investigation Request’. Upon receipt of your Complaint Investigation Request, you will be given a decision as soon as possible. This should be after no more than 20 working days unless there is clearly a good reason for needing more time. If your complaint is complex or needs more detailed investigation, we may look at your complaint immediately at this stage without going through stage 1.

Please start to answer the questions below, to complete the Stage 2 Complaint Investigation Request.
